Resources & Forms

If you are interested in applying for a scholarship, please download the appropriate application. We have separate applications for Associate and Bachelor Degree and for the CDA (there are 2, one for center based providers and one for family child care providers). There is also a separate application for those working at KinderCare and Head Start programs. Please be sure to complete the correct application. All applicants now include the W9 and Direct Deposit form, however they are still here in case you need to update information. Family Child Care providers will need to also download and submit the income form which can be found here. *Those interested in a Bachelor's Degree scholarship must have completed an Associate Degree or at least have 40 transferrable college credits to qualify.
